A FREE-TO-ATTEND one-day workshop on improving outcomes through innovative commissioning of diabetes services.

Population to Patient: the challenges of integrated diabetes care addresses the need to create an end-to-end service that is fit to deliver patient-centred care for each person with diabetes. Such integration will require that clinical teams, commissioners, medicines managers and other service providers are all aligned if effective delivery of diabetes care is to be achieved. This means changes to established cultures and working practices for all professionals across the health care system.
